Logon Dialog - Innersafe for Windows Media Player
NOTE: Screenshots for Media Player Classic look virtually identical.
Passwords are optional in the professional edition because a passfile may be used. The logon dialog will appear when Media Player plays a protected file. The Access Control Dialog can be used to require authentication for other activities if desired (e.g., minimized window restore).
